Client Development Evaluation Report

The Client Development Evaluation Report (CDER) contains diagnostic and evaluation information for persons who have active status in the DDS system. In general, only persons of age three and above have a CDER completed. Prior to age three, an Early Start Report, which contains data items that are more appropriate for infants and toddlers, is completed. A CDER must be completed or updated at the time a person’s Individual Program Plan (IPP) is developed. A new IPP (and thus updated CDER) is required at least once every three years. The CDER should also be updated whenever there is a significant change in the person’s physical or mental capabilities.

The CDER is divided into two major sections (or elements), the Diagnostic Element and the Evaluation Element. The Diagnostic Element contains information pertaining to the individual’s developmental disability(ies), mental disorders, risk factors, major medical conditions, hearing and vision impairments, behavior modifying drugs, special health care requirements, and other special conditions. The Evaluation Element covers information relating to motor, independent living, social, emotional, cognitive, and communication skills.

CDER data is collected by the regional centers, or in cases of persons residing in the developmental centers (DCs), the data is collected by the DCs. CDER files have been archived on a semiannual basis (June / December) since 1979. Beginning in June 1992, the CDER files have been archived on a monthly basis.

As of March 2023, in order to protect privacy, these reports have been removed from our website.  They remain available upon request to datarequests@dds.ca.gov, and can be provided after completing de-identification.  The Department is working on an alternative solution to publishing this information while maintaining privacy protections.  Thank you for your patience.
