Developmental Centers Initiatives Underway
Resident Transition Planning
When a resident of the DC explores a living option in less restrictive environments, the DC is charged with coordinating a personalized plan to prepare for the new environment. Statewide training is provided to staff involved in this process to promote a culture of supporting residents throughout the transition. To enhance the current practices and establish a system-wide standard, the Department is examining its processes for resident transition planning, with an emphasis on person-centered planning, communications with all stakeholders, data collection and management, and documentation. Under review at this time is an analysis of transition management, policies and procedures, and current tools for data, transition planning and diversion. A collaborative plan for each DC implementation will be developed to ensure the safe and successful transition of each individual from a DC/CF and to monitor consistency of implementation throughout the state.
