Program TwoProgram Type:

Nursing Facility (Continuing Medical Services)

Program Goals:

Program 2 works to stabilize, maintain, and/or improve the individual's physical health, which has been jeopardized by acute or chronic debilitating conditions; to minimize the effects of the individual's neuromuscular deficits; and improve sensorimotor development as precursors to active participation in habilitative programming. In addition, the Program assists elderly individuals to experience dignity and comfort in their later years.

Services:

Individualized habilitation programs are developed and implemented by the interdisciplinary team. The primary focus is on improvement of the individual's health status and on minimizing the debilitating effects of neuromuscular deficits through nursing care, restorative care, positioning, range of motion, and equipment adaptations. Individuals participate in a variety of activities, including sensory and environmental awareness; as well as communication, socialization, leisure, and self-help skills development. Special Education, Adult Education, recreation therapy, music therapy, dance therapy, and movement therapy are also provided.

Description of Individuals Served:

Program 2 serves all ages, from children to older adults, all of whom are "at risk" and/or have severe neuromuscular challenges, with chronic, recurring medical conditions. Characteristically, these individuals are non-ambulatory and require total assistance in their daily living activities.
