Program Seven

Program Type:
Eldridge Community Services - Program 7 has four service divisions
Supported Employment Services:
Service Goal: To provide individuals who have developed a successful foundation of vocational skills the opportunity to be employed in a training experience at a community-based site - in a corporation or other business; and to provide these individuals with the services and supports they need in order to further develop their job skills. Services: Supported Employment Services offers community-based opportunities for paid employment through Sunrise Industries, in partnership with local businesses. Sunrise Industries has developed these partnerships to provide qualified individuals with on-site work. Assistive technology and work modifications are available, based on need.
Vocational Services:
Service Goal: To provide a wide array of paid employment options that allow individuals to select areas of work that meet their interests, while providing employees with on-the-job training to develop work skills and prepare for future work opportunities.
Services:
Vocational Services offers a variety of paid employment opportunities through Sunrise Industries. Employment opportunities include, but are not limited to: light assembly, packaging, recycling, plant propagation, letter shop, and janitorial services. Individuals' work stations and work tasks are individually modified with adaptations and jigs based on needs.
Senior Services
Service Goal: To provide the person who has reached age 55 or older and prefers to not work with the opportunity to retire to a day program that is designed specifically for the older individual.
Services: Senior Services provide social, recreational, and leisure services for all individuals who meet the following criteria: 55 years of age or older; have chosen to work reduced work hours or retire from work; have indicated a desire to engage in senior activities; and/or, have a medical condition which precludes them from participating in Adult Day or Vocational Services. Senior activities include arts and crafts, cooking, gardening, exercise, music, table games, relaxation, special events, community inter-generational experiences, and seasonal and holiday events. Environments and activities are modified for the continued success of the individual. Services are provided for people who reside at Intermediate Care Facility residences, as well as Nursing Facility residences.
Adult Day Services
Service Goal: To provide services and supports to individuals that meet the person's choice and preferences, and enable the individual to become more independent in all aspects of life.
Services: Adult Day Services offers a broad spectrum of activities and supports to adults who have physical, non-emergency medical, and developmental challenges. Services include, but are not limited to: development of work skills, exercise, movement, sensory-motor stimulation, restorative care, and leisure. Individuals are provided with the adaptive supports necessary to enjoy a variety of experiences. Adult Education Services are provided in partnership with the Sonoma County Office of Education.
