Eldridge Community Services - Intermediate Care Facility and Nursing Facility Adult Day Programs

Description of Program:

Provide person centered Adult Day programs based on assessment outcomes, priority needs, interdiscipinary planning and anticipated personal life goals. The main focuses are in the three areas. These areas are Adult Education, Habilitation and Training; Vocational Skills Acquistion and Paid Employment; and Senior Living Well and Activity Programs with Community Integration infused into each. Multiple disciplines work together to provide the training and supports that enable each participant to be exposed to new opportunities, develop interests and acquire new skills. Program 7 includes Sunrise Industries, Vocational Support and Technology Services, Adult Educational Services and has an established partnership with Sonoma County Office of Education in the provision of the Nursing Facility Adult Day Programs.
