Case Management

The Habilitation Specialist does not perform individual consumer case management, but is a member of the "habilitation team" established for each WAP or SEP provider, which is composed of the following members:

  • The Regional Center case manager;
  • The WAP or SEP case responsible individual;
  • The WAP or SEP consumer; and,
  • When appropriate, his or her parent, legal guardian, or conservator and any other individual named by the consumer.; and,
  • In cases where the WAP or SEP consumer is also a VR consumer, the VR counselor shall also be included.

The habilitation team shall meet when it is necessary, or if there is a disagreement among the team members on issues or eligibility of services to determine any of the following:

  • If the services available at the WAP or SEP best meet the needs of the consumer;
  • Initial eligibility;
  • Develop an IHC;
  • Continued eligibility;
  • If the consumer would be better served by another WAP or SEP;
  • The appropriateness of job placement.

If the Habilitation Specialist disagrees with the team, the Specialist shall make the final decision subject to appeal to the department by the consumer or his or her designated representative. The Specialist shall substantiate any decision made by stating in writing the reasons for disagreement and the decision and that the decision is not made for fiscal reasons.

Service Coordinator means a Regional Center employee whose primary responsibility includes preparing, implementing, and monitoring consumers' IPP, securing and coordinating consumer services and supports, and providing placement and monitoring activities.

Service Coordination (W&I Code, Section 4640.7) means those activities necessary to implement an IPP, including:
  • Participation in the IPP process;
  • Assurance that the planning team considers all appropriate options for meeting each IPP objective;
  • Securing, through purchasing or by obtaining from generic agencies or other resources, services and supports specified in the person's IPP;
  • Coordination of service and support programs;
  • Collection and dissemination of information;
  • Monitoring implementation of the plan to ascertain that objectives have been fulfilled
  • Assist in revising the plan as necessary.

The Regional Center shall assign a service coordinator who shall be responsible for implementing, overseeing, and monitoring each IPP.

The service coordinator may be an employee of the Regional Center or may be a qualified individual or employee of an agency the Regional Center has contracted with to provide service coordination services, or persons described in W&I Code, Section 4647.2.

The Regional Center shall provide the consumer or, where appropriate, his or her parents, legal guardian, or conservator or authorized representative, with written notification of any permanent change in the assigned service coordinator within 10 business days.

No person shall continue to serve as a service coordinator for any IPP unless there is agreement by all parties that the person should continue to serve as service coordinator.

Where appropriate, a consumer or the consumer's parents or other family members, legal guardian, or conservator, may perform all or part of the duties of the service coordinator described in this section if the Regional Center director agrees and it is feasible.[W&I Code, Section 4647.]
