Your experience in the Early Start Program matters and the Department wants to hear from you!
The Family Outcomes Survey focuses on the helpfulness of early intervention. Your feedback about your experience with the program is crucial so the department can improve the Early Start program.
If you would like to share your experience with California’s Early Start program, please click on the link below to access a brief survey. You will be asked to enter your child’s date of birth, zip code, ethnicity and unique client identifier (UCI) at the top of the form. If you are unsure of your child’s UCI, please reach out to your service coordinator.
Surveys must be completed by October 31, 2025!
Your child’s UCI and date of birth will be used only to verify that they were served in the Early Start program during Fiscal Year 2024 – 2025. Demographic data such as zip code and ethnicity will only be used to check that we receive representative feedback from the families and children we served. We will not share this child-specific data outside of the Department.
