Grant Guidelines

Refer to Attachment A for step-by-step grant application instructions.  The Department has a standardized set of measures that must be used for all community connector projects (e.g. promotora, navigator, and peer/parent mentor). Standardized measures will be added to your grant profile in post-award. Grantees may also collect additional information for these projects. Refer to Attachment B to review standardized measures.
