MLS New Questions

The NCI MLS added 8 questions to the Adult Consumer Survey and 12 questions to the Family Guardian Survey. These questions were added to gather information specific to the transition from the Developmental Center to the community and their experience living in the community.

  1. Has there been a change in any of the medications the person takes since the person moved into the community?
  2. Did people talk about what you wanted at the planning meetings?
  3. Did you feel comfortable and relaxed at the planning meetings?
  4. Did you have homes to choose from when deciding to move?
  5. Did you have day program or day activities to choose from when planning to move?
  6. Do you have the equipment you need in your new home?
  7. What do you like most about your new home?
  8. What do you miss about living at the Developmental Center?

  1. When you first heard that your family member needed to move to a new community home, were you “for it” or “against it”?
  2. How do you like your family member’s new home?
  3. Please list one or more things you LIKE about your family member’s home.
  4. Please list one more things you DON’T LIKE about your family member’s home.
  5. Did you attend the IDT planning meeting?
  6. Did you feel comfortable and relaxed at the IDT planning meeting?
  7. The Interdisciplinary Team (IDT) identified your family member’s services and supports based upon his/her needs.
  8. The IDT identified your family member’s services and supports based upon his/her preferences.
  9. The IDT identified your family member’s services and supports based upon his/her choices.
  10. There were a variety of residential choices available during the IDT meeting.
