If your child received Early Start services between July 1, 2025, and June 30, 2026, we want to hear from you!
The Family Outcomes Survey is your opportunity to share your experience with early intervention services. Your feedback helps us understand what’s working for families and where services can improve. For example, feedback from families in the Family Outcome Survey led to clear, multilingual resources. Some of these included the short animated videos and the Early Childhood & Transition Resource Lookup webpage, that help families easily find information and understand early intervention and transition services.
Access the Family Outcomes Survey to share your experience by Oct. 30, 2026.
You will need your child’s date of birth, ZIP code, ethnicity and unique client identifier (UCI) to complete the survey. If you don’t know your child’s UCI, please contact your service coordinator. This data is used only to check that we receive feedback from a representative sample of the children and families who actually receive regional center services.
