In August, the Department mailed the 2025–26 Family Outcomes Surveys in English and Spanish to Early Start families. Unfortunately, the Spanish version was printed without a survey number or family address.
This information is needed to match your survey response to the correct regional center.
If you fill out the paper survey, please make sure to:
- Return all pages of the paper survey, or
- Write your address on the survey page you send back.
You may also fill out the survey online.
Families will need to provide the child’s unique client identifier (UCI) number to complete the online survey. If you do not know your child’s UCI, please reach out to your service coordinator.
When we prepare the final report, your responses will be combined with the responses of other families. Nothing you share will be linked back to you.
Your feedback is important and helps improve the Early Start Program. Survey responses are due by October 30, 2026.
