Your experiences with developmental services can help shape what’s next.
The California Community Living Network (CCLN) is creating ways to understand what impact developmental services have on you and your family. As part of this work, it is looking for people to help test new questionnaires and provide feedback.
The questionnaires were created as part of the PAVE (Person-centered Advocacy, Vision, and Education) project. This project aims to put the experiences of individuals and families at the center of measuring service quality.
The information gathered will help providers know what they’re doing well and where they can improve.
Who Can Participate?
You may participate if you are age 18 or older and receive one or more of the following services:
- Supported Living Services (SLS)
- Independent Living Services (ILS)
- In-Home Respite
- Supported Employment
Family members and close friends of a person age 4 or older who receives one of the services listed above also can participate.
What Will Participants Do?
Participants will be asked to:
- Complete one or more online questionnaires
- Share feedback about the questionnaires and their experience completing them
As a thank you for participating, you will receive $10 for each questionnaire section completed and be entered into a drawing for a chance to win $250.
