Special incident reports (SIRs) are written to document critical and unexpected events that happen to an individual.  These incidents may signal possible health or safety concerns, or the need for additional services and supports.  Service providers, regional centers and the Department monitor SIRs to identify trends and develop strategies to prevent and reduce risks.

To increase public transparency about SIRs, a new user-friendly dashboard now provides meaningful insights while protecting personal privacy.  In the dashboard, you can view SIR data from the most recent quarter by type of incident, where people live at the time of the incident, and by age and race.  You can interact with the new dashboard and learn more on the Risk Management and Mitigation webpage.
