In 2025, regional centers will start using a new format for Individual Program Plans (IPPs). The new “Your Plan” guide explains the new format and describes what a person-centered IPP is. Translations for “Your Plan” and the new IPP template are underway and will be made available when complete.
Your Plan can help you identify your preferences, strengths, and goals. In the Your Plan guide, you will find:
- An opportunity to learn about Person-Centered Planning
- Tips for before, during and after your planning meeting
- A workbook called “About You” that includes:
- Questions to help you think about the important areas of your life
- Space for you to write down your answers to those questions
Your Plan is an optional guide you can choose to use, or not. You can read and fill out its pages or ask people for help. You can take Your Plan to your IPP meeting. This can help make sure you cover all the areas of your life that are important to you.
If you have questions about the new format for the IPP, please reach out to your regional center. You also can learn more on the IPP page of the DDS website.
