Use “Your Plan” to Prepare for your Individual Program Plan Meeting
Get ready for your Individual Program Plan (IPP) meeting with a new tool called “Your Plan”!
The Department of Developmental Services has created the Your Plan guide to use as you prepare your goals and future plans. Your Plan offers tips for before, during and after your planning meeting, like how to:
- Know your rights and the Individual Program Plan process
- Think and talk about the areas of your life that are most important to you
- Make decisions about the types of service and supports you may need at home, at work, and in the community
A new format for your IPP is being used in 2025. The Your Plan guide can be used to prepare for your IPP meeting. Learn more and download Your Plan today: www.dds.ca.gov/rc/ipp. If you have questions, you can ask your service coordinator for more information about the IPP or Your Plan.
